EMT Medical provides you with quick and convenient online forms so you can personally input or update your medical profile at any time. Your EMT Medical Medical Profile stores your essential medical information online such as medications, allergy information, shot records, and medical and family history. This information gives emergency personnel a comprehensive summary of your current medical conditions and issues within seconds, which can determine the course of treatment provided.

How do my medical and health records get into my EMT Medical account?
Download and complete the Medical Record Release form found on the Summary Page when logged into your EMT Medical Account. Then send the form to your doctor. The form authorizes the release of your health records from your doctor to you. Once your records are received, you simply upload the information into your EMT Medical account where it is immediately ready for your review and emergency access anytime, anywhere.

How do I update my EMT Medical account after I visit my doctor?
Updating records into your EMT Medical account is easy. Simply go to the EMT Medical Members Home Page and download a medical release form on the Summary Page. Send the form to your doctor requesting updated records be sent to you. You will add the new information to your EMT Medical account once it is received. This way, you'll have an up-to-date record of your most current health condition. Remember, you can update your online medical profile information such as medications, current medical conditions, and allergies anytime.

What if my Doctor is not "cooperative" and will not forward my records to me?
Since legally they are your records, your doctor is required by federal law to provide your records to you or your representatives, they may charge a processing fee, but the records are yours.

What if my Doctor charges for copies of my files?
In most cases, the doctor will provide copies of your files at no charge. However, in some cases the doctor may charge you a nominal fee for copies.

May I include other important documents in my EMT Medical account?
You can add important Life Documents to your EMT Medical account. Advanced Directives include:
- Living Will
- Healthcare Durable Power of Attorney
- Medical Authorization to Treat a Minor
- Organ Donation Form
To include these documents in your electronic health record, simply mail copies of these documents along with a form you download from the member website. The form is located on the Summary Page of the member pages.

How is my online medical information accessed in an emergency?
In an emergency situation medical providers access your account using the Life Number printed on your EMT Medical Condition Access Card. Note that entry through the Emergency Login only gives medical personnel the ability to review your online medical information, not to add to it or make changes to your profile. Only your username and password may authorize changes.

Do emergency personnel have to subscribe to the EMT Medical service to access my personal health record?
No. Only you need to subscribe to the EMT Medical service. In an emergency situation any medical providers with access to the Internet can login to your account using the Life Number printed on your EMT Medical Condition Access Card (your personalized emergency ID card). Unlike other medical information management systems, physicians do not need any special equipment or have to be members themselves to access the EMT Medical service. Note that entry through the Emergency Login only gives medical personnel the ability to review your file; they will not be able to alter your personal health records. Only your username and password may authorize changes.

During the enrollment process, you choose in advance those you want notified in case of an emergency. Emergency personnel can use the information in your EMT Medical account to contact them.

How safe are my electronic health records at EMT Medical?
EMT Medical has taken every precaution to ensure that electronic health records are secure, remain confidential and that individual privacy is respected. All data is backed up routinely and stored at an offsite location. In addition, your information is only available via a password protected member area, which assures integrity with 256 bit encryption of all information passed to the client.

How do I make sure that only authorized emergency personnel or myself view my medical profile?
The username and password that you choose during the enrollment process will ensure that you are the only person who can update your account. The Emergency Login information provided on your EMT Medical Condition Access Card (Life Number) allows authorized emergency personnel to view your records only. No other person can gain access to your information through any other means.

What should I do if my EMT Medical card is lost or stolen?
Like your ATM or credit card, if you lose your EMT Medical card just contact our Customer Service at 800-891-7636 and we'll cancel the Life Number on your card and issue you a new one. You can print a temporary card with your new emergency login pass codes from the "Member Profile" section and keep it with you until your new permanent card arrives in the mail.
